📌 Important Reminder:
Texas is currently accepting reevaluation documents through April 30, 2026, and all DBE/ACDBE firms must complete this process to maintain certification.

📑 Required documentation includes:
• Form CIV-PN (Personal Narrative) with supporting documentation
• Personal Net Worth Statement
• Most recent personal tax return (with all schedules and W-2s)
• Current personal bank statement
